This page is about the inquiry form on this website. It isn't my Notice of Privacy Practices, which is a separate document I give you if we start working together.
What the form asks for
Your name and date of birth, how to reach you, whether you're asking for yourself or someone else, how you plan to pay, what brings you here, how you found me, and anything else you'd like to add. Only your name and email are required.
Where it goes
Into my practice's Google Workspace account. It doesn't pass through the company that hosts this website, and it isn't stored anywhere else. I read every inquiry myself. The person who maintains this website can also reach that account in order to keep it working, under a signed confidentiality agreement.
How long I keep it
If we start working together, your information moves into my clinical records. If we don't, I clear your inquiry after six months.
What this site doesn't do
It sets no cookies. There's no analytics, no advertising, and no tracking of any kind. Nothing on the page is loaded from another company, and nothing you type is saved in your browser.
If you want your inquiry deleted
Email me and I'll delete it. You don't have to give a reason.
